Reduced Lung Sliding in the setting of COPD Exacerbation and Hyperinflation
60 yo w SOB and hypoxia. Hx COPD.  Lung ultrasound at multiple rib spaces looked like this. Lung sliding is difficult to appreciate, making pneumothorax a consideration. But there are vertical artifacts near the rib shadows. Considerations include tiny PTX w double lung point sign, hyperinflation in COPD exacerbation, bleb. 
With multiple rib spaces like this and hx of COPD, reduced lung sliding due to exacerbation rises. Remember not all absent lung sliding = PTX.
