Airway Intubation Checklist
GATHER/TEST EQUIPMENT: NC, BVM + PEEP Valve, Free flowing IV, EKG, SpO2 monitor, ETT x2 sizes, Blade x2, OPA, Suction, Video scope, LMA, Ventilator, Cric kit, Bougie, Don PPE, Capnograph
PLAN
 o Assess for difficult airway (LEMON)
 o Anticipate risks (HOpI killers)
 o Approach: RSI / DSI / Awake
 o Pre-Medication and Paralytics
 o Primary and secondary airway plan
 o Emergency plan/Cric preparations
PREPARATION/INDUCTION
 o Position patient, adjust height of bed
 o De-nitrogenation
 o Push medications and wait
VISUALIZATION/TUBE PLACEMENT
 o Insert Laryngoscope
 o “Call the view” and suction if needed
 o Adjust view if needed
 o Place tube, withdraw stylete
CONFIRMATION
 o Auscultate
 o Capnography
 o Repeat DL/VL if uncertain
POST INTUBATION MANAGEMENT
 o Secure ETT
 o Reassess hemodynamics and oxygenation
 o Analgesia/Sedation plan
 o Ventilator settings
 o Connect in-line suction
 o Place NG/OG Tube
 o ABG (ideally at least 10 min post intubation)
 o Chest radiograph (ideally post NG placement)
 o HOB > 30 degrees
