Giant dilated left atrium on Echocardiogram (a4c)
It belongs to a 92-year old lady with a history of hypertension and a pacemaker. MR was only mild.
Mitral valve doesn’t look typical rheumatic and patient is in sinus rhythm...
1. This can be LA aneurysm idiopathic.
2. Long chronic severe MR 
3. ASD with R/L shunting which is rare.
