Complex Aortic Atheroma on Suprasternal Notch View 
Never forget the suprasternal view during standard TTE examination.

Would you anticoagulate such patient if in sinus rhythm?
Yes, plus statin in all cases.  In addition, if bleeding risk is not high, aspirin on top on anticoagulation until the follow up echo (usually within a month or so). Most of them silently disappear, sometimes w/o therapy.
