Transjugular Intrahepatic Portosystemic Shunt (TIPS)
TIPS involves the creation of a low-resistance pathway between a hepatic vein and an intrahepatic branch of the portal vein, using an expandable metal stent. This offloads the portal venous system, reducing portal pressures.
Procedure is performed percutaneously, usually by interventional radiology.
Indications:
 • Recurrent or refractory variceal hemorrhage
 • Refractory ascites
Contraindications:
 • Active infection / sepsis
 • Severe coagulopathy and/or thrombocytopenia
 • Portal vein thrombosis
 • Hepatic encephalopathy
 • Heart failure
