Hemolytic Anemia - Differential Diagnosis Algorithm
Hereditary:
 • Peripheral blood smear + DAT + further History
 • Bite cells, Heinz bodies, DAT (-) → G6PD Deficiency
 • Sickle cells, DAT (-) → Sickle Cell anemia
 • Hypochromic & microcytic RBCs, DAT (-) → Thalassemia
 • Spherocytes, DAT (-) FHx? → Hereditary spherocytosis
Acquired:
 • Spherocytes, DAT(+) → Immune mediated hemolysis (idiopathic, malignancy, autoimmune disease, drugs, transfusions)
 • Schistocytes DAT(-) → Microangiopathic hemolytic anemia (TTP, HUS, DIC, eclampsia, malignant HTN)
 • Recent travel/Fevers, DAT (-) → Infection mediated (Babesiosis. Malaria, Clostridium)
