Amiodarone Thyroid Complications
Amiodarone: 
 - iodine-rich, similar structure to T4, with half life up to 100d
 - inhibits conversion ofT4 to T3
 - can prevent T4/T3 from entering peripheral tissues
 - can be directly toxic to thyroid follicular cells
 • Amiodarone Induced Thyrotoxicosis Type I - Typically in pts w/ latent thyroid dz: iodine load leads to excess thyroid hormone synthesis (Jod-Basedow phenomenon)
 • Amiodarone Induced Thyrotoxicosis Type II - Typically in pts w/ normal thyroid: amiodarone leads to destructive thyroiditis
 • Amiodarone Induced Hypothyroidism - Typically in pts w/ underlying thyroid dz: unable to "escape" from the Wolff-Chaikoff effect (iodine load suppresses thyroid hormone production and release)
