HAS-BLED Score - Risk of Major Bleeding in 1 Year
Characteristics (1 point each)
 • Hypertension: Baseline SBP > 160mmHg
 • Renal disease: Cr > 2.6 mg/dL, on HD, or s/p renal transplant
 • Liver disease: Cirrhosis, or total bilirubin > 2x ULN, or AST/ALT/Alk phos > 3x ULN
 • History of stroke
 • History Of major bleeding
 • Unstable INR: Time in Therapeutic Range < 60%
 • Age > 65
 • Concurrent use Of antiplatelet meds or NSAIDS
 • Consumption Of ≥ 8 alcoholic drinks/week
