Streptocccus Pneumoniae
Microbiology: Gram Positive, Alpha-hemolytic, Catalase Negative
Epidemiology - 10% adult carriers:
 • Most common cause of: CAP, Bacterial meningitis, Otitis media
 • Important cause of sinusitis, septic arthritis, osteomyelitis, peritonitis, and endocarditis.
Diagnosis:
 • Physical Exam, CXR, ENT exam, CT sinus
 • Sputum culture (Sensitivity 60%, Spec > 95%)
 • Urine antigen in CAP
 • Urine antigen in bacteremia (Sensitivity 82%, Specificity of 97%)
Treatment:
 • B-Lactams (Penicillin/amoxicillin if sensitive)
 • Macrolide (addition might be associated with improved mortality in patients with bacteremia)
Duration: PNA 5 days, Bacteremia 10-14 days, Meningitis 10-14 days, Endocarditis 4 weeks
