Risk factors:
 - Low CD4 count
 - Disseminated OI
 - Paucity of inflammatory response to OI (in CM)
 - Short duration or OI treatment
Possible mechanisms:
 - Uncoupling of  innate and acquired immunity
 - Restoration of exuberant pathogen-specific cellular immune responses
 - Defective or delayed regulatory responses
