Platypnea-Orthodeoxia Syndrome on Physical Exam
The case of a patient with a diagnosis of cirrhosis and platypnea-orthodeoxia syndrome.
There is notable positional dyspnea and hypoxemia. Symptoms occur when the patient is upright and resolve when supine.
You can observe how the respiratory mechanics improve markedly in the supine position. In addition, there is a drop in SaO2 from 96% to 90% (and in paO2 from 78 to 66 mmHg) when passing from the supine position to the sitting posture. No murmurs on physical examination.
