Adrenal Insufficiency Treatment - Stress Dose Steroids
• Large Bore IVs, ~2L IVF & Draw workup labs
• 100mg Hydrocortisone IV x 1, 50mg q6hrs afterwards
• Treat Adrenal Insufficiency before concurrent hypothyroidism
• Treat underlying stressors & correct electrolyte abnormalities
Sick Day Rules
• Mild Stressor (URI, fever, minor surgery with local anesthesia)
- 2-3x basal for 2-3 days
• Mod Stressor (general anesthesia surgery)
- Hydrocortisone 45-75mg daily
• Major Stressor (critical illness)
- Hydrocortisone 150-200mg/day
