Urine Microscopy - Casts
Acellular Casts
 • Hyaline Casts - Consists of only precipitated Tamm-Horsfell protein without other constituents. Non-specific, but can be seen in dehydration.
 • "Muddy Brown" Casts - Strongly suggestive of acute tubular necrosis (ATN)
 • Waxy Casts - Non-specific; seen in a variety of acute and chronic kidney diseases
 • Fatty Casts - Contains yellow-tan fat globules. Strongly suggestive of nephrotic sydnrome
 • Pigment Casts - Contains one of several colored compounds (e.g. heme, bilirubin, etc...)
 • Granular Casts - Believed to be from degeneration of a cellular cast
Cellular Casts:
 • RBC Casts - Strongly suggestive of glomerulonephritis
 • WBC Casts - Strongly suggestive of interstitial inflammation (which can be either infectious or non-infectious)
