Autoimmune Hemolytic Anemia (AIHA) Treatment
Warm AIHA:
1) Steroids - 1-2 mg/kg prednisone daily
• Until Hgb > 10, no more hemolysis
• Med duration 8.5 mo!
2) +/- Rituximab, esp if severe disease
If relapse, not responding > 3 weeks of steroids: Ritux, IVIg, Splenectomy, MMF, cyclophosphamide, azathioprine
Cold AIHA:
• Warm the patient
• If you need to temporize: Plasmapheresis or IVIg
• Polyclonal:
- Infection → support, often self limited
- Rheum → treat underlying condition
• Monoclonal:
- Rituximab, Bortezomib, Bendamustine
