Acute Monocular Vision Loss - Differential Diagnosis Framework
• Cornea: Keratitis
• Anterior Chamber: Hyphema, Acute angle closure glaucoma, Anterior Uveitis
• Vitreous Humor: Vitreous hemorrhages, Endophthalmitis
• Optic Nerve: Optic neuritis, Papilledema, Ischemic Optic Neuropathy (GCA, CRVO, CRAO)
• Optic Chasm: Rapidly expanding pituitary path (infection, inflammation, bleed)
After the optic chiasm, visual loss will be bilateral homonymous hemianopsia and cannot account for monocular vision loss.
