Iron Deficiency in Heart Failure
Pathophysiology:
Chronic heart failure leads to an increase in inflammatory cytokines → Inflammation causes an upregulation of Hepcidin which degrades ferroportin → Decreased ferroportin leads to decreased iron absorption and iron release
Diagnosis:
     Ferritin <100 ng/mL (absolute ID) OR
     Ferritin 100-299 ng/mL with TSAT <20% (functional ID)
Treatment - IV Iron:
 • Several studies have shown significant improvement in symptoms and QOL and decreased hospitalizations
 • Two meta analyses showed reduced risk for the composite endpoint of all-cause mortality and CV hospitalization
