Eosinophilia - Differential Diagnosis Algorithm
Idiopathic Eosinophilia
Primary Eosinophilia:
• Primary hypereosinophilic syndrome
• Eosinophilic Leukemia
• MPN/MDS w/ eosinophilia
• Familial/Genetic: Hyper-lgE (Job) syndrome, familial eosinophilia
Secondary Eosinophilia:
• Neoplastic: Solid tumors (esp. GI, lung, skin), Heme malig (AML, CML, T-cell leuk/lymphoma, B-cell lymphomas), Systemic mastocytosis, Langerhans cell histiocytosis
• Allergic: Asthma, Eczema, Drug Hypersensitivity (DRESS, drug-induced eosinophilia, ABPA), Gleich syndrome (episodic angioedema w/ eosinophilia)
• Infectious: Loeffler syndrome, Helminths (Strongy, Schisto, hookworm, filariasis, trichinella, ascariasis), Scabies, Fungi (Cocci, Aspergillus), Viral (HIV, HTLV-1), Mycobacteria (TB, Leprosy), Bacterial (syphilis, bartonella)
• Rheum/AI: EGPA, IgG-4 disease, Behcet disease, Sarcoidosis, SLE, Eosinophilic esophagitis, Eosinophilic pneumonia, Adrenal insufficiency, Chronic inflammatory disease: e.g. IBD
