Horner's Syndrome on Physical Exam

A 75yoF was referred in for facial swelling and eyelid changes 1 month following neck surgery.  She demonstrated the classic miosis and ptosis (both upper and lower) of the right eye consistent with a diagnosis of Horner's syndrome.

This condition may be the result of damage along the sympathetic nervous system from conditions such as a carotid artery dissection, tumors along the apex of the lung (Pancoast tumors), and other space occupying lesions within the neck. In our experience, the cause is more commonly iatrogenic or induced by treatment/surgery; as in our case where she recently underwent corpectomy to decompress the spinal cord. The video highlights how the right pupil is not dilating properly in the dark due to the interruption in the sympathetic response.
