Unilateral Floppy Eyelid Syndrome

This middle-aged male reported to clinic for persistent blurry vision and irritation of the left eye and eyelid. He mentioned that his lid would self-evert at night and be very swollen in the morning. He reported sleeping on his left side and snoring loudly, as well as feeling tired frequently. A sleep study was recommended and confirmed he had sleep apnea. He had unilateral SPK on the left eye consistent with his level of decreased vision. He also had two large “pyogenic granulomas” on the superior temporal palpebral conjunctiva. This is a great example of why checking eyelid laxity is so important and can help solve some patients symptoms.

Our management for this patient was:
1. Order a sleep study
2. Recommend night-time goggles from eyeeco.com
3. Start artificial tears
4. Start generic FML (fluoremethalone) BID for 1 month due to presence of the large “pyogenic granulomas”

Surgical evaluation is possible if patients desire, but a conservative approach is recommended first. We generally recommend that sleep apnea is under control for 3 months prior to any surgical eval.
