Dacrolith Eyelid Expression 
Chronic canuliculitis typically presents with recurrent unilateral epiphora and a pouting punctum. Other signs include mucus discharge, localized conjunctival and medial canthal irritation. Common causes are buried punctal plugs and bacterial infections. Dacroliths can form which are “concretion-like” sulphur granules associated with the bacteria Actinomyces. These sulphur granules can make local and oral antibiotic treatments relatively ineffective. Treatment often involves removal of the dacroliths, which can sometimes be done in the office but usually requires a canaliculotomy. Fortunately for this patient, her symptoms resolved completely after in office expression only.
