Hippus on Physical Exam
Hippus: alternating dilation and constriction of pupils independent of illumination, convergence or psychic stimuli. Hippus is a rhythmic, spasmodic but irregular phenomenon. The term of hippus derived from the Greek word of horse; maybe because of its rhythmic nature resembling horse galloping.
Hippus is usually is a normal phenomenon but pathological hippus occurs due to hepatic or renal encephalopathies, MS, MG, trauma, altered mental status due to frontal lobe dysfunction, and every disease that affects the autonomic system such as parkinson disease, MSA, PSP, aconite poisoning. In a hospitalized patient, hippus has a poor prognostic value.
