Approach to Gout
Diagnosis: r/o septic arthritis! Needle-shaped, negative birefringent crystals
Treatment:
 • Weight loss, diet, d/c culprit medications
 • Acute: NSAIDs, Steroids, Anakinra
 • Colchicine: Acute and chronic treatment
 • Chronic - Urate Lowering Therapy: Xanthine Oxidase Inhibitors (Allopurinol, Febuxostat), Pegloticase
When to start Urate Lowering Therapy (ULT):
1) 2+ flares/year
2) Gouty tophi
3) Radiographic Damage
4) Urate nephropathy, renal stones
