Whipple Disease (Tropheryma whipplei) - Diagnosis and Management 

Caused by: Tropheryma whipplei

Symptoms and Signs:
1. Migratory poly/oligoarthralgia (80%)
2. Weight loss (90%)
3. Abdominal pain; colicky (60%)
4. Intermittent diarrhea (75%)

Diagnostic Criteria - Either one of:
 • Oculomasticatory/oculo-facial-skeletal myorhythmia OR
 • PAS-positive macrophages from a small bowel biopsy OR
 • Two different positive T. whipplei tests (PAS, PCR, or IHC) from the same specimen OR
 • One positive T. whipplei test (PAS, PCR, or IHC) from two different specimens
