Hemimasticatory Spasm on Physical Exam Hemimasticatory spasm is a disorder of the trigeminal nerve → involuntary jaw closure due to spasmodic unilateral contraction of jaw-closing muscles- masseter and temporalis → similar to hemifacial spasm. Our patient had a normal MRI & responded to clonazepam / trihexyphenidyl. Often mistaken for Hemifacial spasm. Myokimia will cause such gross movement and trismus as in our patient.
