Convergence-Retraction Nystagmus on Physical Exam

Convergence-retraction nystagmus: irregular, jerky nystagmus, associated with convergence and retraction of both eyes, especially on attempted upgaze. ? damage to the supranuclear fibers, which have an inhibitory effect on the midbrain convergence or divergence neurons.

Excessive & sustained discharge from the medial rectus and other extraocular muscles. CRN  localizes to the dorsal midbrain & is usually part of Parinaud's syndrome. Dorsal mesencephalic syndrome and acute hydrocephalus after subarachnoid hemorrhage
