Iron Deficiency in Congestive Heart Failure (CHF)

Iron deficiency (ID) in heart failure is a common comorbidity affecting up to 50% of patients, leading to:
 ↑ HF Hospitalization
 ↓ Quality of life and hospitalizations survival
 ↓ Exercise capacity

Diagnosis:
 • Ferritin < 100 ng/mL OR Ferritin 100 - 299 ng/mL + TSAT < 20%

Risk Factors:
 • Female sex
 • More advanced stages of HF
 • Higher NT-proBNP and CRP

Ganzoni Equation:
Iron deficit (mg) = [body weight (kg) x (target Hgb — actual Hgb) x 2.4 + depot iron]

Treatment Options (IV Iron Preferred)
 • Ferric carboxymaltose - 750 mg
 • Ferric gluconate - 125-150 mg
 • Ferumoxytol - 510 mg
 • Iron dextran - 100 mg
 • Iron sucrose - 100-200 mg
