Anti-IgLON5 Encephalitis - Ocular Examination
Female with 6 months h/o excessive daytime sleepiness, abnormal movements, dysarthria. On examination- neck drop, choreiform movements. PSP like phenotype (vertical ophthalmoplegia), severe dysarthria, dysphagia progressing over 6 months. Serum, CSF ++ for IgLON5. MRI normal. IgLON5-Neuroinflammatory +neurodegenerative disorder.
Clinical pentad of sleep disorders (parasomnia/ sleep breathing difficulty), bulbar syndrome (dysphagia, sialorrhea), cognitive decline, movement disorders (chorea), or a progressive supranuclear palsy (PSP like) syndrome.
