Tongue Fasciculations in Kennedy's Disease

A man in his 40’s with progressive nasal twang, dysphagia and weakness climbing stairs.  Exam from the front shows a pectoral crease (sign of trapezius weakness & gynecomastia). Final Dx- Kennedy’s disease: X-linked slowly progressive  adult-onset disease characterized by muscle weakness - Spinobulbar distribution & Gynecomastia . Treatment is symptomatic and supportive, and life expectancy is normal.
