Thrombus in transit seen in RA. dilated RV with McConnell's sign. Pt had extensive bilateral PTE. Started on heparin then taken for EKOS catheter-directed thrombolysis, aspiration thrombectomy. Discharged 3 d later with normal LV and RV function.
