Left Main Coronary Artery Thrombus seen on Angiography

“Mother of all coronary thrombi”.  55 year old male with Anterior wall STEMI, in Cardiogenic shock, ECG predicted Left main occlusion, taken for Primary PCI, first injection purposefully in root. Thrombus protruding from LMCA into aortic root. 

Patient was in shock with VT storm. Just wired, dilated, flow established, but died on table
