Renal Artery to IVC Arteriovenous Fistula on Angiography

44 yr male, DOE, bounding pulse, BP 150/68, mid syst murmur, Echo done twice- normal. LVDD was 60, IVC distended, CI calculated 4.6 L. He had past laparotomy for stab injury.  High output HF due to IVC renal artery fistula!

I saw the laparotomy scar and asked him about it, he reluctantly told the history of stab, then I examined abdomen and heard the loud continuous bruit/hum. Ultrasound showed renal artery pseudo aneurysm communicating with IVC. Angio and CT showed complex anatomy. Surgery done.
