Physicians should consider other disorders that may arise from or mimic acute pyelonephritis (Table 312). Intrarenal and perinephric abscesses, which usually are complications of acute pyelonephritis, are more common than emphysematous pyelonephritis, which is a necrotizing infection that produces intraparenchymal gas within the kidney that is identifiable by renal imaging. This disorder occurs most often in older women with diabetes.
