 - Plaque rupture/erosion with non-occlusive thrombus 

Myocardial Infarction Type 2:
 - Atherosclerosis and oxygen supply/demand imbalance 
 - Vasospasm or coronary microvascular dysfunction 
 - Non-atherosclerotic coronary dissection 
 - Oxygen supply/demand imbalance alone
