Hyperkalemia, K > 10

I recently was called for a STEMI with this ECG. Pt was 40’s male without CP/SOB who presented with weakness. Acute diagnosis?

Key to this ECG is widening of the QRS and the hyperpeaked T waves. Imagine a hook pulling the T wave up almost off the top of the page. When you see this...start treating hyperkalemia even before you get labs back. And consider a call to your friendly neighborhood nephrologist.
