80F w AFib w pacemaker on warfarin presents with n/v x 4. Initial BP 100/40, HR 85, then pt becomes altered, hypotensive w MAP 50s, HR 115. Check out this parasternal short axis view.

Fibrinous pericardial effusion w tamponade. RV collapsed around pacer wire. INR >12. Pt treated w Vit K + FFP, taken emergently to cath lab. >700 ml of bloody fluid removed. Pt discharged in good condition 4 d later. Other causes of fibrinous effusion: TB, cancer, bact infection.
