 - Symptoms: Melena, hematemesis, hematochezia 
 - Reason for admission (if different) 
Date: 
 - Vitals 
 - Notable physical exam findings (rectal exam) 
 - CBC 
 - Coags, LFTs, lipase 
PMH:
 - Focus on prior GI/Liver History 
 - Prior endoscopy & endoscopist 
 - GI surgeries 
 - Other PMH: Cardiac/pulm/oncologic 
Meds:
 - Antiplatelet & AC/ NOACs 
 - NSAlDs 
Course:
 - NPO status, time of last meal 
 - Treatment started (PPI IV, octreotide, antibx) 
 - Transfusions
