Hoffmann's sign was originally described as follows: the test is performed by supporting the patient's hand so that it is completely relaxed and the fingers are partially loose. The middle finger is firmly attached, partially extended, and the nail is scraped with the examiner's fingernail. The clicking must be done with considerable force, even to the point of causing pain. The sign is present if rapid flexion of the thumb and forefinger occurs. Sometimes nails other than the middle finger are selected for clicking.
