Anticoagulation for Atrial Fibrillation - CHA2DS2-VASc

I always tell my students, one of the first thing you do when you see a patient with atrial fibrillation in clinic is to calculate a CHADS-VASc score! This determines whether or not they should be on anticoagulation therapy for stroke prophylaxis. We have 2 current guidelines that are used in practice, CHEST and ACC/AHA/HRS. Both guidelines recommend DOAC over warfarin as first-line for non-valvular AF, but they do differ on CHADS-VASc cut-offs. CHEST recommends anticoagulation for patients with at least ONE non-sex risk factor, while ACC/AHA/HRS recommendation is for patients who have at least TWO non-sex risk factors.
