Landolfi’s Sign on Physical Exam

A 73-year-old man presented with shortness of breath. On ocular exam, pupil dilation and constriction that were synchronized with his heartbeat were noted, a finding known as Landolfi’s sign. He was found to have severe aortic regurgitation.

In severe aortic valve regurgitation, Landolfi’s Sign refers to systolic contraction and diastolic dilation of the pupil and results from the large stroke volume present in this disease state.
