Landolfi’s Sign on Physical Exam A 73-year-old man presented with shortness of breath. On ocular exam, pupil dilation and constriction that were synchronized with his heartbeat were noted, a finding known as Landolfi’s sign. He was found to have severe aortic regurgitation.
