Kussmaul's Sign on Neck Physical Examination
Elderly woman with a history of myocardial revascularization surgery is hospitalized for heart failure syndrome. Normally, venous pressure should drop at least 3 mmHg with inspiration. The Kussmaul sign is defined by an increase (or lack of fall) of jugular venous pressure with inspiration and is classically associated with constrictive pericarditis, although it has been reported in patients with restrictive cardiomyopathy, massive pulmonary embolism, right ventricular infarction and advanced left systolic heart failure. It is also a common and isolated finding in patients after cardiac surgery without other hemodynamic abnormalities.
