Tubal Ectopic Pregnancy on Transvaginal Pelvic Ultrasound

A 31yo female s/p tubal ligation presents with right lower quadrant pain and lightheadedness that occurred acutely 1 hour prior at work. What is the best next step?
Resolution: Surgical Intervention. There is a large (8wks ga!) right tubal ectopic pregnancy with hemoperitoneum appreciated on the sagittal view, indicating rupture. Methotrexate is contraindicated. A FAST or urine will not change management and only delay care.
