Low-dose aspirin and low-molecular-weight heparin in lupus and Antiphospholipid Syndrome (APS) pregnancy

• No previous history of thrombosis or pregnancy complications BUT high-risk aPL profile:
  Aspirin (75-100 mg/day)
• Previous history thrombotic APS (arterial or venous): consensus +++
  Aspirin (75-100 mg/day) + curative low-molecular-weight heparin (LMWH)
• Previous history of obstetric APS: EULAR Recommendations 2019
  - History of 23 recurrent spontaneous miscarriages weeks of gestation OR fetal loss > 10 weeks of gestation : Aspirin + preventive LMWH, continued for 6 weeks after delivery
  - History of delivery <34 weeks of gestation (eclampsia, preeclampsia or placental insufficiency): Aspirin ± preventive LMWH, continued for 6 weeks after delivery
  - History of "non-criteria" obstetric APS: Aspirin ± preventive LMWH, continued for 6 weeks
after delivery
