Portal Venous Gas on Point of Care Ultrasound (POCUS) secondary to Necrotizing Pancreatitis

Adult male with abdominal pain/hypotension.  RUSH performed.  Perihepatic window shown.  Diagnosis/Differential?
Answer: Portal venous gas. Age dependent differential. Adult-abdominal catastrophes (ischemic bowel, perf ulcer, intra-abdominal sepsis). Also seen s/p CPR as well as COPD, steroid usage.
