Arterial Bleed after Trocar Insertion

This appears to be a pig abdomen, but this case highlights the importance of visualization and technique during port placement, especially in midline.

- Control always - dominant thenar eminence on top of port with index as a secondary blocker, primary blocker non dominant index and thumb. Once see tip, angle away from critical structures. 
- This is completely preventable for a couple of reasons: This is a bladed trocar (took me a few tries to pause it, but definitely bladed), and trocar should never “pop” through. Always controlled, holding trocar with nondominant index and thumb during placement prevents a “pop.”
