Pupillary Hippus

The pupillary hippus is defined as a continuous oscillation of the pupillary diameter in the absence of variations in light or other external stimuli. Its pathophysiology remains questioned, but it seems to be related to the dysregulation of pupillary motiliy mediated by the Edinger-Westphal nucleus, the parasympathetic accessory nucleus of the oculomotor nerve, activated through the insula. The patient in the video had a decreased level of consciousness and the hippus was the clue for the diagnosis of non-convulsive status epilepticus.
