Seesaw Nystagmus

Sawtooth or seesaw nystagmus is an unusual but characteristic type of nystagmus. Typically there is twisting and elevation in one eye and simultaneous extortion and depression in the other eye. Nystagmus can be pendulum or jerk. The pendular shape is commonly due to a mesodiencephalic mass in the midline, bilaterally compressed. The jerk form is mainly due to a unilateral lesion at the mesodiencephalic junction. A current theory presupposes a unilateral lesion of the interstitial nucleus of Cajal, sparing the rostral interstitial nucleus of the medial longitudinal fascicle. Another concept suggests an injury to the vertical vestibulo-ocular reflex. We have varied causes, such as trauma, stroke, multiple sclerosis etc. The video was provided by friend and colleague @pedrov.chaves, resident of Santa Casa SP.
