Liver laceration and pneumoperitoneum on POCUS

BAT s/p MVC-unrestrained driver. RUQ pain. Good quality FAST exam windows negative. Look at detailed view (sag/trv) of RUQ. Findings?
Liver laceration present.  U/S has lower sensitivity for detection of solid organ injury.
Patient did have trace amount of free fluid adjacent to the liver laceration but would not have been detected on a focused perihepatic window.
Always look for second injury.  Patient also has pneumoperitoneum from duodenal perforation. Note the presence of ringdowns (red arrow)
