Managing warfarin INR Warfarin dosage must be individualized according to the patient's response to the drug, and adjusted according to the results of the INR. For most indications, the usual INR target is 2.5 (range 2-3) for most conditions. Over-anticoagulation can lead to severe bleeding and hemorrhage, while under-coagulation can result in thrombosis.
