Gout: Pathogenesis and Clinical Findings
- Conditions of rapid cell turnover: hemolysis, psoriasis, lymphoproliferative disorders
- Medications (ASA, ACE- inhibitors, cyclosporine, diuretics)
- Alcohol Intake -> Alcohol is -> catabolized into ketones and lactic acid -> Ketones and lactic acid compete with uric acid for excretion in urine, excretion of uric acid
-> Incr uric acid concentration in blood, easily leaks into joints -> Uric acid crystallizes in a peripheral joint -> Activation of complement cascade results in leukocyte chemotaxis -> Neutrophils enter joint to phagocytose the crystal -> Needle-shaped uric acid crystals puncture neutrophils, releasing degradative enzymes into joint space -> Chronic tophaceous gout
