Emphysema, when it is severe, can mimick Langerhans cell histiosytosis. When it extends beyond the centrilobular area to the edge of the secondary lobule, it may look as if it is cystic with walls. In patients with LCH, the pathologist may find LCH, but also areas of emphysema, respiratory bronchiolitis and even fibrosis.
